Name Change

If you wish to submit a name change, please submit a signed letter of request which includes

Your current name
Your folder number or social security number
Your address
Legal proof of your name change.
Acceptable legal proof includes copies of:

Marriage certificate
Pages of divorce decree showing name change
A clear copy of a valid driver's license showing your new name

IF you wish to receive a copy of your license reflecting the name change, include a duplicate license fee of $15 made payable to the Board of Educational Examiners. Please print legibly or type the letter of request. Then submit the information to:
