Name Change

Name changes may be updated at the time of renewal by noting the name change on the application.

If you wish to submit a name change at another time, please submit the following:

* A letter of request which includes-

      your current name

      your folder number or social security number

      your address

      legal proof of your name change.

IF you wish to receive a copy of your license reflecting the name change, include a duplicate license fee of $15 made payable to the Board of Educational Examiners.

Please print legibly or type the letter of request. Then submit the information to:
