Duplicate of License:
If your license or certificate was lost or ruined, you may apply for a free duplicate. Use one of the forms below instead of clicking on the link on the Board of Educational Examiners website for "Order a Duplicate of your License."
Renewal Concern:
If you are renewing your license and your classes were cancelled due to flooding and you need more time to finish your renewal credits, please submit the following information with the renewal application: (1) the credits you have completed, (2)a letter from the AEA verifying the renewal class was cancelled due to flooding and (3) your anticipated completion date of the renewal credits. You do not need to send a fee with the application.
If you are renewing your license but you were not able to renew because of flooding or the tornado, please send the following information with your renewal application (1) the credits you have completed, (2) a letter from your administrator verifying the area was affected by floods (3) the anticipated completion date of the renewal credits. You do not need to send a fee with the renewal application.
